Dear Orange /Ulster Boces Teachers,

 

As of  September 1, 2008, Vision World has become the exclusive provider for Ulster/Orange BOCES Teachers for their Optical services.

 

Within this new contract members have the option of adding eligible family members at the cost of $4.50 monthly through payroll deduction.  This Deduction will be taken pre-taxed from payroll. Simply fill out the enrollment form attached.

 

Let’s review the optical benefit we are providing, first of all, you are entitled to:

A yearly Comprehensive eye exam

Single vision, bifocal, trifocals, safety or oversized lenses

A $120.00 retail frame allowance within the Eyewear Collection

Cosmetic or Sunglass tints

Standard Hard, Soft Daily Wear or Extended Wear Spherical Contact lenses are available in lieu of glasses 

 

If you loose or break your glasses you are covered under the Loss Protection Program.  At time of loss or breakage a fee of $25.00 will be collected.  This will allow you to receive glasses covered within this program.  Any questions regarding this benefit please call. 

 

Please fill out the attached enrollment form. Please have this form returned by October 1, 2008.  The faster this form is returned the faster the benefit will go into effect for your dependents.
